As a part of our Club's general shirt order, we have the opportunity to order PERSONALISED SHIRTS for you to buy and keep.
The deadline for orders is Midnight on Sunday 10 January 21.
The shirts are made by Dynasty and supplied by Go Hockey.
Top: Mens Cut, Middle: Women's Cut, Bottom: Personalisation
The Men's shirt sizes are: S, M, L, XL, 2XL, 3XL
The Women's shirt sizes are: 8, 10, 12, 14, 16, 18, 20
If you would like to order a PERSONALISED SHIRT for you or a family member then please send the following information by eMail (as soon as possible) to: abchockeyclub@gmail.com

Then the Shirt/s detail (please repeat for multiple/family orders):
The Name You Want Printed on your shirt (Please check the spelling as this can't be changed once produced!)
The format should be SURNAME or SURNAME G (you can have an optional 'first name' initial when ordering a family set)
Please do not request 'nicknames' or 'abbreviations'
The shirt Size you want to order:
The Fitting (eg. Mens or Women's):
Your Preferred Number: Both Men's and Women's shirt numbers range from 3 to 59... please take your pick! (Your choice will be checked, verified for duplication in each squad and confirmed back to you).
All numbers are issued on a first come first served basis).
Your Payment of $50 per shirt should be made by Bank Transfer, using your order reference (name and mobile number) to the normal Club Account: 12-3035-0551332-00 at the earliest time please.
To guarantee your order this payment should be before the ORDER DEADLINE of Midnight on Sunday 10 January 21.
